Steroid metabolome and multiple pregnancy

Steroid hormones can be divided into 2 groups - intracellular and extracellular, depending on location of their activity. Intracellular ones influence expression of genes while acting as transcription factors. This so-called genomic effect is very slow. Extracellular (non-genomic) steroids bind to neurotransmitter receptors located on the cytoplasmic cell membrane and thus affect ionic channels permeability. Their effect is faster. We refer to them as neuroactive steroids (produced in different body tissues or administered externally) or neurosteroids (synthetized in nervous system cells). Some neuroactive steroids and their metabolites (e.g. progesterone) are crucial in stabilizing the pregnancy. Other steroid disorders play their role in wide spectre of pregnancy complications, such as preterm labor, preeclampsia, intrahepatic cholestasis in pregnancy etc. Our scientific interest in collaboration with the Department of Steroids and Proteofactors of the Institute of Endocrinology in Prague is focused on investigation of multiple pregnancies in term of steroid metabolome. The Specifics of Speech Development in Premature Infants

The diploma thesis deals with the problems in clinical speech therapy and is entitled Specifics in the speech development of premature infants. The theoretical and practical findings are included in the thesis. The theoretical part is devoted to premature birth, risks associated with premature birth and care of premature individuals. It also describes multiple pregnancy, twin pregnancy and twins in terms of their development, education and specific forms of communication. The last chapter of the theoretical part deals with child development, speech development and communication disorders. In the practical part, the empirical part, the research methods of the qualitative investigation and its focus are described in detail. The main aim of the thesis and sub-objectives are given. The thesis concludes with a summary of the data collected, which was used to develop a grounded theory. 4 Abstract This diploma thesis is concerned with modern methods of treatment for infertility. It examines to what extent and in what cases are these methods used and what risk does it pose for a woman's and a child's health. Paticular emphasis is placed on the methods of assisted reproductive technology as the most used, effective and the most invasive. Nowadays 10-15% of couples face the problem of infertility. In most of the cases this problem can be solved with the help of hormonal therapy, surgery or assisted reproductive technology. The most common complication of ART (assisted reproductive technology) is ovarian hyperstimulation syndrome which leads to higher frequency of miscarriages and can be fatal for a woman. Since more that one embryo is usually transferred in ART children conceived with the help of this method can face different problems related to multiple pregnancy such as higher risk of preterm birth, higher risk of congenital disorder or intrauterine growth retardation. Nursing process in a woman with monochorionic twin pregnancy.

The topic of this bachelor thesis is monochorionic twin pregnancy. The thesis is divided into a theoretical and a practical part. The theoretical part briefly introduces different types of twins. Then it concerns itself with monochorionic twin pregnancy ̶ with prenatal care, typical complications and risks related to this type of pregnancy and also with timing and conduction of labour. In the practical part of this thesis, we are presented with a case study of monochorionic diamniotic twin pregnancy complicated by twin to twin transfusion syndrome (TTTS). The case study contains basic identification of the pregnant woman, her medical history, a description of her hospital stay and a nursing plan for the pregnant woman. The objective of this thesis is to comprehensively describe monochorionic twin pregnancy. The practical part then aims to illustrate the hospital care and the execution of a nursing plan on a case study of a woman diagnosed with monochorionic twin pregnancy. The nursing plan is executed in line with the Gordon's functional health patterns model. This thesis is intended for midwifery students and midwives interested in this matter. Development of communication abilities in premature infants

The presented thesis deals with the topic of development of communication abilities in preterm infants from the perspective of special education. The thesis is for clarity divided into several parts. In the first part of the text the issue of incidence of premature births in the Czech Republic is discussed, following subsections are devoted to the definition and distribution of characteristics bonding with immaturity of child and approach selected health problems associated with postpartum adaptation and subsequent psychomotor development of child. The following chapter which analyzes the system of care for premature newborns in the Czech Republic focuses on two forms of care, acute care and followed-up. Next part has been devoted to issue of premature infants from logopedical perspective and describes the physiological process of development of communication abilities of child and based on current knowledge, especially foreign research, analyzes the peculiarities of speech development of preterm infants. The last and key part of the work, based on case studies, analyzes the specifics of development of communication abilities of four originally extremely and very preterm children of multiple pregnancies in the background of the their overall psychomotor development. Nursing care about women with multiple pregnancy

This bachelor thesis is aiming on multiple pregnancy problematic. The document has two parts, the theoretical and practical one. Introduction chapter in theoretical part, is focusing on epidemiology and creation of multiple pregnancy, I continue with prenatal care and diagnostic, which is targeting on ultrasound, screening, invasive examination and schedule of prenatal care. The next theme is problematic of risks and complications within multiple pregnancy, which may occur in multiple gravidity. At the end of theoretical part I describing managing of twins labour and multiples. In practical part I am dealing with nursing procedure of woman with multiple pregnancy, where I analysing specific case. In this thesis is detailed processed nursing procedure, which is integral part of nursing care. Output of my bachelor thesis is recommendation for practice, given to the all midwives. The thesis as a complex is determined to midwives and students of midwifery. Key words: woman, multiple pregnancy, nursing care.
Multiple-births in Czech Republic

The main goal of this thesis is the analysis of multiple-births in Czech Republic between the years of 1950 and 2005 including the international comparison and assess of the influence of assisted reproduction on occurrence of multiple-births. The thesis is divided into five different parts. The first part is dedicated to the analysis of fertility according to a succession of births in Czech Republic while using the indicators of total fertility, the average age of mothers in the time of childbirth and the total number of new-born babies. The second part focuses on multiple-births considering both historical and contemporary point of view and on possible risks for both the mother and the foetus. Next part describes the assisted reproduction, its history, present and commonly used methods. The fourth part is dedicated to the influence of assisted reproduction on the occurrence of multiple-births and to the analysis of multiple-births with the use of absolute and relative number of mono-births and multiple-births indicators, indicators of total fertility, masculinity index, the rate of stillborn babies and the proportion of children born outside of marriage. The last part compares the development of multiple-births within the European countries according to NIDI 1999 classification.
